DPS or FoMF for GvG?
Now I've done abit wondering, and have come to the decision that I want to ask you :P
Death Pact Signet or Flesh of My Flesh?
DPS for low casting time and 100/100 status. But death if the one you res' dies.
FoMF if for decent status when revived but no penalty when the ne you res'd dies.
Of course, if your team cant keep a player alive for 2 mins, then you're fail, but I still wanted you opinions =)

1 FoMF and 1 DPS and then the rest Res Sigs.

on of the frontliners usually brings a dps, and a midline (mesmer,ele or paragon) brings fomf.

It depends, if you have dual frontline normally the Hammer/ 2nd War will take DPS and a person on the midline will take FoMF, preferably the mesmer.
